Provider staff shall indicate the primary contact and other pertinent treatment team members within the clinical record. -
For directly operated providers using the Integrated Behavioral Health Information System (IBHIS), this shall be done using the Client Practitioner Assignment Form. -
The primary contact may be any appropriately designated staff by the program manager. -
There shall be no more than one (1) primary contact designated per provider, and the provider in which the primary contact works shall be indicated in the clinical record. -
Provider staff shall document and maintain the current name and contact information of other relevant service providers within the clinical record including, but not limited to, the client's primary care provider. -
For directly operated providers using IBHIS, this shall be done using the Outside Providers Form. -
All coordination activities shall be accomplished using permissible communication methods (e.g., telephone, email, fax, direct message) and documented in the clinical record in accordance with DMH Policies 401.02 and 401.03. | |
