Clinic staff shall dispose of each medication in the sealed pharmaceutical waste receptacle and record the disposal on the DMH Pharmacy Disposal Medication Log. Clinic staff shall keep the original for three (3) years. -
DMH Pharmacy technician shall separate the medications as follows: -
Unwanted, unused, or expired medications purchased through the DMH pharmacy wholesaler account shall be quarantined and shipped to the reverse distributor for processing. -
In the event packages containing unwanted, unused, or expired sample medications and the patient’s own medications are received by DMH Pharmacy Services, the DMH pharmacy technician shall quarantine, separate, and ship the medications to the pharmaceutical waste management vendor in accordance with DMH Policy 306.15. -
The pharmacy technician shall schedule a pickup date and time with the County contracted reverse distributor at least every two (2) months. -
When the reverse distributor credit report becomes available, the DMH pharmacy technician shall check the credit against disposal records and identify credited items. -
The DMH pharmacy technician shall identify the credit memo number and amount of credit issued for each credit memo on the reverse distributor report and reconcile it with: -
Checks issued to DMH by manufacturers or the reverse distributor; and -
Credits posted on the DMH pharmacy wholesaler account. -
The DMH pharmacy technician shall ensure credits are received by DMH through one (1) of the two (2) methods. -
Checks and credits shall be sent to accounting for processing. -
The DMH pharmacy technician shall contact the reverse distributor for credits not received and continue to follow up with the reverse distributor until credits are issued. -
Any credits not collected shall be reported to the DMH Chief of Pharmacy and accounting. | |
