I.  PURPOSE
 
To establish procedures for security of medications stored in the Los Angeles County Department of Mental Health (DMH) directly operated clinics.
 
II.  DEFINITIONS
 
No definitions are associated with this policy.
 
III.  POLICY
 
All medications shall be stored in a secured, locked medication room at all times, not accessible to clients or unauthorized staff.
 
Medication room keys and access cards shall be unique to each medication room and shall be separate from other keys or access cards in the building.
 
Under no circumstances shall medication room keys or access cards be duplicated without written authorization from the DMH Chief of Pharmacy.
 
The DMH Chief of Pharmacy shall designate nurse(s) responsible for the security of each clinic medication room.
 
No client medication shall be stored in any clinic office, group room, treatment room, or location other than the DMH-designated medication room.
 
Only individuals designated by the DMH Chief of Pharmacy shall have unsupervised access and possession of the medication room key or access card.
  • Each clinic shall maintain a current list of authorized personnel on record and filed with the DMH Chief of Pharmacy office.
     
  • Housekeeping staff shall enter designated medication rooms and perform their duties only under immediate and continuous visual observation by authorized personnel.
     
  • The key or access card shall be returned to the DMH Chief of Pharmacy or designee upon termination of employment at the clinic, and the employee name shall be removed from the clinic’s authorized personnel list.
     
  • Any lost key and access card shall be reported immediately to the Office of the DMH Chief of Pharmacy.
All clinics that store medications shall be inspected for medication room security at least quarterly by the DMH Chief of Pharmacy/designee in accordance with DMH Policy 352.04 Clinic Medication Inspections and Audit Control.
