  Policy 100.06 Mental Health Services Act Issue Resolution Process
 
  PROCEDURES
  1. Issues shall be addressed and resolved with an emphasis on increasing service quality.
    1. If a Mental Health Services Act (MHSA) planning, implementation, or operations issue cannot be resolved by the MHSA Administration Division, the issue shall be reviewed by the Department’s appointed Community Leadership Team (CLT). 
  2. To file an MHSA program issue, the requester shall:
    1. Follow the process put in place by the contractor;
      1. The contractor shall maintain a log including:
        1. Nature of the issue
        2. The disposition of any investigation
        3. The outcome
    2. Use the MHSA Issue Resolution Form; or,
    3. File in person at 510 South Vermont Avenue, 1st Floor, Los Angeles, CA 90020. ATTENTION: MHSA Issue Resolution.
  3. The MHSA Administration Division shall investigate the issue. 
    1. The Requestor shall receive a notification of resolution in writing.
  4. If the Requestor has exhausted the Issue Resolution Process without satisfactory resolution, they may submit an MHSA appeal in writing, by email, or by calling either the Department of Health Care Services or the Mental Health Services Oversight and Accountability Commission:
