All Critical Laboratory Values shall be submitted electronically AND called into the Ordering Clinic. -
Once a call or voicemail is received by the facilitator, they shall record the laboratory call-back number and immediately locate and notify Authorized Personnel, in-person. Calls shall not be transferred blindly to Authorized Personnel. If Authorized Personnel is not available on-site, but can be reached by other means, then the facilitator shall provide Authorized Personnel with laboratory call-back number and receive verbal or written confirmation that notification has been received. Otherwise, facilitator shall proceed to notify the next Authorized Personnel as sequenced in the outline below. -
The facilitator shall first try to notify the Ordering Prescriber to receive and address the Critical Laboratory Value. -
If the Ordering Prescriber is not available, the facilitator shall notify the Supervising Psychiatrist. Supervising Psychiatrist is responsible for receiving the Critical Laboratory Value and addressing or triaging as needed. -
In the case the Supervising Psychiatrist is not available, the facilitator shall notify a nurse. The nurse is responsible for receiving Critical Laboratory Value and ensuring the laboratory result is addressed by a prescriber as soon as possible. -
If all parties listed above are not available and if the facilitator is not a program manager, they shall notify the Program Manager. The Program Manager shall then facilitate by contacting any DMH covering Authorized Personnel within two (2) business hours to take a call from laboratory personnel or have Authorized Personnel call the laboratory to obtain Critical Laboratory Value. -
Authorized Personnel, other than Ordering Prescriber, in receipt of Critical Laboratory Value information from Laboratory Contractor, shall follow procedures set forth below to provide the information to Ordering Prescriber - Obtain and record the following
- Name of laboratory personnel reporting the Critical Laboratory Value
- Date and time call is received
- Name of Client
- Date of birth of Client
- Ordering Medical Staff
- Critical Laboratory Value with Read-Back
- Confirmation electronic Critical Laboratory Value has been received and posted in OrderConnect
- Authorized Personnel in receipt of Critical Laboratory Value, once in possession of above information, shall complete the following immediately and no later than three (3) business hours from notification by clinic staff:

Inform the Ordering Prescriber or Designee of Critical Laboratory Value, which includes a secure email to both the Ordering Prescriber or Designee and Supervising Psychiatrist alerting them of Critical Laboratory Value. The secure email shall include the client’s name, date of birth, and date and time the results were received. -
Before the end of the business day Authorized Personnel in receipt of Critical Laboratory Value shall follow-up with Ordering Prescriber or Designee to confirm the Critical Laboratory Value was received. -
Ordering Prescriber or Designee shall check the client’s record in OrderConnect to obtain additional information. Ordering Prescriber or Designee shall document a Medication Service Progress Note with the specific action taken for Critical Laboratory Value in the Client’s medical record. The critical value shall be addressed within one (1) business day upon receipt of the result by the Ordering Provider or covering Designee. | |
