  Policy 200.01 Advance Health Care Directives
 
  PROCEDURES
  1. Staff shall provide all Medi-Cal beneficiaries, 18 years of age and older, with written information regarding Advance Health Care Directives. 
     
    1. Information shall be provided at the time of the first face-to-face or telehealth service contact and thereafter upon request.
       
    2. Staff shall provide information either by paper, email, or mail.
       
    3. Staff shall scan completed Advance Health Care Directive Acknowledgement (MH 635) into the beneficiary's clinical record.
       
    4. Staff shall scan any completed, properly executed Advance Health Care Directive into the beneficiary’s clinical record.
       
    5. Staff shall document in a prominent part of the current health care record whether or not an Advance Health Care Directive has been executed.
       
  2. Patients' Rights Office shall update written information regarding Advance Health Care Directives provided to clients to reflect changes in State law within 90 days.
