I.  PURPOSE
 
This policy outlines the responsibilities of directly operated programs in relation to notification of changes to a program (i.e. new, relocation, closure) and changes to the Professional Director (i.e. Supervising Psychiatrist) or Administrator (i.e. Program Manager).
 
II.  DEFINITIONS

III.  POLICY
 

Senior Deputies, Deputy Directors, District Chiefs, Program Managers, and Supervising Psychiatrists are responsible for notifying Pharmacy Services Chief and Pharmacy Supervisors by email of the following at their first awareness. 

  • New program establishment 
  • Program relocation to new address 
  • Medication room relocation to new room at same address 
  • Closure of program 
  • Anticipated departure of supervising psychiatrist from their role at the licensed clinic 
  • Anticipated departure of professional director from their role at the licensed clinic 

Senior Deputies, Deputy Directors, and Chief Medical Officer are responsible for identifying interim personnel to carry out duties of departing Program Managers and Supervising Psychiatrists and notify Pharmacy Services Chief and Pharmacy Supervisors as soon as identified. 
 

Supervising Psychiatrists and Program Managers are responsible for posting Board of Pharmacy issued Clinic Licenses at a location that is viewable to the public.
 

Program Managers are responsible for notifying Pharmacy Services Chief and Pharmacy Supervisors by email of any correspondences received from licensing and regulatory bodies, e.g. California State Board of Pharmacy and California Department of Public Health, within one (1) business day of mail arrival at program.
