I.  PURPOSE
 
The purpose of this policy is to identify those records that comprise the Designated Record Set.
 
II.  DEFINITION
 
No definitions are associated with this policy.
 
III.  POLICY
 
The Los Angeles County Department of Mental Health (DMH) shall identify those records that comprise the Designated Record Set. 
 
The Designated Record Set includes the following:
  • Clinical and billing records about individuals maintained by or for a covered health care provider;
  • The enrollment, payment, claims adjudication, and case or medical management record systems maintained by or for a health plan; or
  • Information used, in whole or in part, by or for the covered entity to make decisions about individuals.
Designated Record Set may be disclosed or amended in accordance with the DMH Policy 501.06.

For Directly Operated providers, the existence of a Designated Record Set is dependent upon the opening of a clinical record using the LE00019 episode with the exception of DPSS co-located programs.
