| | This policy establishes the members, responsibilities, and authority for Los Angeles County Department of Mental Health (DMH) Pharmacy and Therapeutics (P&T) Committee.

| | III. POLICY
| Members of the P&T Committee include: - Appointed Psychiatrist(s) (voting member)
- Chief of Pharmacy (voting member)
- Chief of Nursing or designee (voting member)
- Representative from administration
Responsibilities of P&T Committee include: - Establish and maintain a formulary of medications for DMH;
- Act as the sole and final governing body over policies and procedures related to procurement, storage, distribution, dispensing, administration, monitoring, and usage of medications, medication processes, medication safety, information technological advances in pharmacy as well as pharmacy, pharmacy technician and pharmacist practices;
- Perform drug utilization evaluation (DUE);
- Provide guidance to the Department with regard to relevant reportable outcomes and quality measures;
- Review significant medication errors; and
- Communicate and disseminate educational material to clinical staff regarding pharmacy and medications.
