An evaluative counseling session should be held and performance appraisal forms should be completed at the end of an employee's initial six month probationary period and during the annual performance evaluation cycle. The purposes of the performance evaluation program are to provide an opportunity for discussing goals for improved performance and to create a record about performance which may be considered in promotions, transfers, or other employment decisions.

Completed performance appraisals for Campus employees (non-hospital) should be uploaded to the performance evaluation application.  See the UAB Performance Management webpage for details.  

Performance evaluations for UAB Medicine employees are to be completed and maintained in the UAB Medicine electronic evaluation system.
